Where can I get records of proof for my vaccine shots?

I got accepted to John Jay College, and they are requiring proof of immunization records. I am 21 years old, and I can not find my records. I am not sure about going to my doctor because I moved to another state, so he does not have my records. I was born in American; so, any ideas?
